A form is a poor way to describe a cat that has stopped eating and a worse way to arrange a goodbye. Both of those are phone calls, and I would rather you rang.
Weekdays. If I am at a visit, leave a message and I ring back between houses.
If your animal is in trouble now, do not wait for me. Go to an emergency hospital — I am one person driving between appointments and I cannot come today.
The form is for the rest
Wellness visits, vaccinations due, a second animal added to an existing appointment, or a question about whether a home visit is even the right thing for what is going on.
